• Open the form by clicking on icon. Save the form to your device.
• Open saved form using any Adobe software (Reader, Acrobat, etc.).
  • If you don’t have it, Adobe Reader is a free software you can download and instal on your device
• Form has fillable fields where you can type your answers. Please fill the form and save it for your records
• Click on SUBMIT button at the end of the form. If your device has default email client:
  • An  email will open with   pre-filled  To:, Subject: and Text: with your form as an attachment. Please do not change the addresses but you can edit the Text or add other addresses
  • If required, you can attach additional supporting documents (such as Insurance, Proof of Residence, Registration, etc.)
  • Send the email
• If your device has no default email client, SUBMIT button may not function. In this case, create a new email manually and
  • In To: field enter the recipient, e.g. memberrecords.halloween@gmail.com or [email protected]
    • In Subject: type the form  name. e.g.  “2025 Summer Storage Application Form”
    • In Text: enter whatever text you want to send to HYC
    • Attach the filled in Form as well as any supporting documents required with it   (such as Insurance, Proof of Residence, Registration, etc.)
    • Send the email

For the limited time Members can also fill the forms online and print them, or print the blank forms and fill them manually, and then mail them or hand deliver them to the club with the other required supporting documents and payments.
